Extranet User Acceptable Use Policy

As an extranet user of the portal and subsequent portal sites, I acknowledge I have read and agree to follow the site use conditions of the portal. This includes (but is not limited to):

  • I will NOT share my login name or password with others under any circumstance.
  • I will NOT share LHIN or HSSOntario confidential or private organizational information without written permission of the site lead.
  • I commit to re-setting my password within 7 days of receipt of this email
  • Any content I post to this site will meet site guidelines.
  • I will NOT use a personal or public computer or device at any time to access and contribute to the site.
  • Content will NOT contain Personal Health Information or Personal Information
  • I will gain consent of the individual prior to posting any personal information to the portal site. This includes personal contact information, photos or videos
  • Content will be relate to the business of the LHIN's / HSSOntario and be in the best interests of the same
  • Content will not divulge or contain sensitive or confidential information which is not intended for the audience which has access to the site.
  • I will inform the site lead if I no longer require access to the portal for any reason, including change or loss of employment, change of job function, or completion of work on a project.

Extranet User Guidelines

Authorized access and termination

  • Access to the portal is authorized by the LHINs and or HSSOntario. We reserve the right to suspend or terminate your use of this extranet at any time for any reason.

Personal Health Information & Personal Information

  • Personal Health Information and Personal Information are NOT to be posted on the portal at any time.
  • I will gain consent of the individual prior to posting any Personal Information to the portal site. This includes personal contact information, photos or video's of any person(s).

Computer & Password Security

The Extranet provides access to business documents and it is important to ensure that the documents stored are reliable and do not pose a risk to anyone using them.
  • It is strongly recommended that you access the Extranet from a trusted computer. A work or home computer could be considered a trusted computer because you have the ability to ensure that it operates in a reliable and secure manner.
  • If you're using a trusted computer ensure that the following is present:
    • Anti-Virus software installed and updating on a daily basis
    • Operating system updates are updated regularly
    • Using a firewall or connect via router to your Internet Provider
    • No software from dubious sources (as it may contain malware)
  • Computers in general are known to get infected with malware that can harmful effects (capture/share your username and password; block your access; etc). Public computers (Internet cafes, libraries, hotels, etc) are exposed to a lot of documents, web sites and software on a daily basis that can affect how they operate. If you must access your Extranet from a public computer, please change your password as soon you have access to your regular computer.
  • If unsure that certain content can be stored on the Extranet, please contact your LHIN/HSSO site administrator.
  • Do not share your username and password. Also, keep your password different from your other accounts. This ensures that if your credentials are compromised, other accounts are not affected and vice-versa.
  • If you suspect that your account has been compromised, contact the Site administrator to temporarily disable your account or reset your password.
  • Save your files to a secure location such as your file share, an encrypted USB key, or archive it using a password.
  • If you password-protect files (either Office documents or ZIP archives), please ensure that it is a strong password and that it is communicated to the intended parties directly. Never store any passwords on the Extranet.

Confidentiality

  • The documents, files and information on the Portal are confidential and should not be distributed or disclosed, except to those person authorized to receive and read them. If you are unsure as to whether a document can be distributed, please check with the site owner or administrator.

User conduct

  • This extranet should only be used for lawful purposes and in a manner that does not infringe or restrict the use of this extranet by, any other persons. Always respect the dignity of others and do not post comments that may be deemed offensive.
  • You must not upload, post, transmit or distribute any documents, files or information: (a) in which you do not own the intellectual property rights; or which contains material which is threatening, abusive, libelous or defamatory, pornographic or otherwise obscene, or racially or ethnically or otherwise objectionable; We reserve the right to remove any material that we deem to breach these requirements.
